RESOLUTION

To work around this, please ensure each virtual disk has a unique name by following the following tutorial from VMware, then replicate the system again, and re-try the deployment:
https://kb.vmware.com/selfservice/microsites/search.do?language=en_US&cmd=displayKC&externalId=1002491

CAUSE

This is caused by multiple disks for the machine being deployed having the same file name, for example
[datastore1] VMName/disk0.vmdk
[datastore2] VMName/disk0.vmdk

Because both of these files have the same disk name, when the conversion is attempted it will fail because it is trying to convert two disks with the same name.
